IHF PATIENT SUPPORT SERVICES
We provide support online, by phone and in person to more than
4,**Apply on the website** people affected by heart disease and stroke
and their carers. The key elements of the IHF?s patient support
services are:
* A phone-based programme (The Stroke and The Heart Connect Service)
providing social, emotional and practical support mainly to newly
discharged stroke survivors and people living with a clinical
diagnosis of heart failure.
* Weekly/monthly online peer support and exercise groups.
* A range of complementary programmes including fatigue management,
psychological wellbeing groups, physical activity, counselling and
self-management
* Social media groups providing peer support along with
comprehensive health and wellbeing information and advice
THE ROLE
The Irish Heart Foundation is seeking a highly motivated person with
relevant experience to deliver in-person, phone and online supports.
This work involves working with people to achieve their recovery goals
so they can manage their conditions to live as well as possible.
The role will also involve coordination of monthly face to face stroke
support group meetings and online peer support groups.
Other aspects of the role involve supporting group volunteers as well
as building the profile of the IHF heart and stroke services amongst
key stakeholders.
KEY RESPONSIBILITIES ?
Undertake specific tasks in relation to the Connect phone support
service (SCS), including:
* Work with clients towards their recovery goals.
* Provide a kind and supportive listening environment for each
client, whilst being mindful of empowering the client to manage their
own recovery.
* Networking/signposting to other healthcare professionals and
organizations providing support services.
* Carry out benchmarking with clients at the start and end of the
phone support intervention, using simple outcomes measurement tools.
* Monitor and report on clients? progress weekly and follow
procedures to flag changes in key needs.
* Follow escalation protocols should there be issues or concerns
related to a client?s health or any other circumstances.
* Sign-off the client at the end of the intervention and make
recommendations on ?next step? support programmes, be they IHF or
other agency interventions.
* Work towards building the profile of the SCS service amongst
stakeholders such as other healthcare professionals and key referral
sources.
ONLINE SUPPORT INTRODUCTION: Via Zoom, the IHF runs a series of online
exercise classes, information and social events.
In line with IHF policies and procedures, the successful applicant
will coordinate a range of Zoom sessions, working with other members
of the patient support team according to the team roster and undertake
tasks including:
* Work within the team to recruit expert speakers in accordance with
member requests and needs.
* Supervise weekly exercise sessions delivered by physical activity
trainers.
* Monitor attendance and report on attendance post event
* Monitor the wellbeing and encourage attendee participation during
sessions.
* Work with colleagues to input content and monitor Facebook groups.
OVERSEEING VOLUNTEERS
* Support e-volunteers on the weekly exercise zoom sessions as
required.
* Provide support and direction to support group volunteers,
liaising closely with National Volunteer Co-Ordinator
* Identify and refer clients for the Peer to Peer and Befriending
services, if appropriate
ONSITE IN-PERSON 2-HOUR MONTHLY SESSIONS IN CASTLEKNOCK, WHITEHALL AND
MULLINGAR/MIDLANDS
* Plan and operate 2 hour monthly support group meetings in North
Dublin and Mullingar
* Provide a safe, welcoming and supportive environment for stroke
survivors to interact and support each other and have access to
information and advice that will empower them to live well as well as
possible with their condition.
* Guide and support the volunteers assigned to these in-person
groups. Ensure their availability in advance of group meetings and set
tasks for each session. Liaise with the National Volunteer Manager in
relation to issues and concerns and recruitment of new volunteers.
* Arrange booking of venues, and complete venue risk assessments in
relation to all areas of safety and wellbeing.
* Maintain attendance records and expenditure records.
* Ensure all health and safety measures are in place, including
infection prevention and control and emergency care plans.
* Ensure equipment in use at the group is clean and well maintained
(e.g., Boccia set and the AED).
* Facilitate group sessions to meet service goals.
* Organise occasional recreational outings and carry out venue risk
assessment.
* Be the point of contact for group members between meetings should
they have additional support needs.
* Share information with members regarding the IHF?s phone and
online supports and provide guidance to members as to how to engage in
these services.
* Ensure members are aware of other local supports with regard to
their stroke and social isolation, such as ABI Ireland and Headway,
ALONE and Social Prescribing.
ADMINISTRATION
* Client record keeping ? ensuring all key information is kept up
to date.
* Work sensitively with clients to administer brief service
evaluation tools in order to capture key information tracking service
impact.
* Provide regular reports on all areas of role.
* Occasional input into applications for funding, IHF media
commentary, newsletter and other communications to clients and to the
wider general public.
* Recruit clients for occasional research studies and coordinate
their participation.

HEALTH AND SAFETY
* Undertake Training in First Aid and administer First Aid if
qualified to do so.
* Working with volunteers, support group clients and facilitators to
ensure a safe environment for all.
* Adhere to the IHF Health and Safety policy and procedures at all
times.
TRAINING AND DEVELOPMENT
* Undertake Training as required by the Manager

SKILLS AND EXPERIENCE
* Minimum Qualification: Level 7 (Diploma & Ordinary bachelor?s
degree) or and or relevant experience working with people with
disabilities /neurological conditions.
* A person-centred approach is essential along with understanding of
the principles of inclusion and diversity.
* Confident and comfortable communicating by phone and running
online group meetings with clients with cognitive and communications
challenges.
* Confident and comfortable working with clients with a wide range
of physical, emotional and psychological needs
* Excellent computer and administration skills and a high level of
proficiency using the Microsoft Office suite, particularly Excel and
MS Word.
* Previous experience working with a CRM or patient management
database is desirable but not essential.
* An ability to work on own initiate as well as part of a team.
* Experience working and networking/signposting to other health
professionals and organizations providing support services for people
with disabilities.
* Experience in group facilitation and peer support is desirable,
but not essential.
* A reliable and flexible approach.
* Full, clean driving license and own car and flexibility to travel
on a monthly basis.

DETAILS OF ROLE AND APPLICATION PROCESS
This is a full-time permanent role, Monday to Friday. There will be
travel around Dublin North regions and surrounding Counties.
This role also involves travel on a monthly basis to Mullingar to run
a face-to-face meeting for our members in this region
